All contents of the application must be typed and your name on every page. Applicants can submit the application online via the Upload Scholarship Forms page. Applications can also be delivered to the J. F Maddox Foundation (220 W. Broadway Street, Suite 200, Hobbs, New Mexico 88241-2588). Please DO NOT attach any additional information to the application. Only the inforamtion contained in the application and the specified components will be provided to the Selection Committee for consideration.
